Coach, Parent & Volunteer Information

Thank you for giving your time to help Providence recreation programs be successful. Below are several pieces of information regarding child safety. Please read through these documents and return to this page as needed. 
 

The purpose of this Child Safety Policy is to ensure the safety of Providence City Recreation patrons, under 18 years-of-age, by stipulating proper qualifications and training for the staff and volunteers who manage and operate the City’s facilities and programs. Please read this document and remember these policies as you prepare for your season.

As required by state law, ALL coaches, employees and volunteers are required to submit annual background checks. Providence City will cover the cost of these checks. Before your season begins, please go to coachbackground.com. At the top of the screen, click "Apply for a Background Check" next to "Coaches & Volunteers." You can then search by Association (Providence City Corporation) or by entering prov-coach in the Access Code box. These checks need to be done annually with the calendar year. You will also be required to submit a photo of your driver's license. 

Please fill out the Background Check Release Form and return it to the Providence City office or by email to meganmccuistion [at] providence.utah.gov (meganmccuistion[at]providence[dot]utah[dot]gov).

Before the start of your season, please familiarize yourself with the Providence City Concussion and Head Injury Policy. Should an athlete have a concussion, they must be immediately removed from play. Medical clearance from a qualified health care provider is required for the athlete to return to play. A parent/guardian of each player on your team must sign and return the Providence City Concussion and Head Injury Policy prior to competition.

Be aware of the signs and symptoms of concussions and other head injuries. You can read through this Concussion Information packet or visit www.cdc.gov/heads-up for more information. 

After you have read through the above documents, please sign the Coach/Volunteer Agent Acknowledgement form. It can be returned directly to the city office or by email to meganmccuistion [at] providence.utah.gov (meganmccuistion[at]providence[dot]utah[dot]gov). Thank you!
